The aircraft was undergoing landing gear maintenance at America West's facility in Phoenix, Arizona, when the nose landing gear collapsed. The two mechanics aboard the aircraft at the time were injured, and the aircraft suffered serious damage. Not visible in the photos is a cherry picker which punctured the First Class fuselage.
