| Accident Description: | The aircraft was on an ILS approach to land at Nagoya when the First Officer accidently pressed the TOGA (take-off go around) button located on the forward edge of the throttles. The F/O then attempted to correct the situation by disconnecting the auto-throttle and taking over speed control manually. The TOGA switch, however, also commands a pitch up attitude for a go-around, and as the crew pushed the yoke the autopilot trimmed the aircraft up to the maximum allowable. The aircraft stalled due to the severe nose high attitude and crashed. Crew error in not disconnecting the autopilot and manually landing the airplane. |
